Urinary incontinence, that is, involuntary leakage of urine, affects up to 30% of women in the United States and is expected to grow exponentially as the population ages. Usually classified by type, it can be devastating to quality of life and overall wellbeing. The three main types include:
In the interest of full disclosure, I was immersed in the topic for almost 5 years; I was acting in an advisory capacity to Merck Consumer Care who recently received marketing approval for?Oxytrol For Women. Now that that conflict is gone, I am finally able to share information, and more specifically, whether or not urinary incontinence is linked with menopause and hormone deficiency as estrogen starts to decline. ?Reports suggest that roughly 8% to 27% of menopausal women suffer.
Until now, only a few studies have truly focused on women between the ages of 45 and 60, i.e. what I often call the ?menopausal set.? More importantly (and what you need to know): when French researchers conducted a thorough review of 29 studies, culled down from an initial total of 488, they found that the connection between menopause and urinary incontinence is tenuous at best.
In fact, in one of the largest studies that they looked at ? the Study of Women?s Health Across the Nation (SWAN, which followed 1,529 women annually for 6 years? they found that the data show that during perimenopause, women have a one and a half times greater risk for urinary incontinence, but this risk disappears when they reach menopause. In an associated study, SWAN researchers reported that data linking menopause to a worsening of incontinence symptoms appears to be due to weight gain and not hormones. Ironically, other studies have linked the decline in estrogen to improvements in urine incontinence symptoms.
So, what about HRT? Where does it fit into the picture if the link between menopause/estrogen decline is weak? Well, when the researchers looked at that data, they found that type of incontinence and how HRT is administered does matter. Both the Nurses Health Study and the esteemed Cochrane Database have shown that oral HRT increases incontinence risk and worsens symptoms, while topical treatments may actually improve symptoms and number of episodes.
The researchers write that menopause has little if any impact on the risk of urinary incontinence in general, especially when other factors such as weight gain or age are taken into account. The good news for women with stress incontinence is that the condition appears to decline after menopause, although, granted, a mixed bag may take its place. And oral HRT? Buyer beware! If you?ve got either stress incontinence or overactive bladder, it may worsen your symptoms!
Urinary incontinence may be as much as taboo subject as vaginal atrophy. But it?s definitely one that needs to be talked about, especially as more and more women transition into midlife. Fortunately, it looks like menopause is not the cause, which should bring a sigh of relief. Meanwhile, gotta go? Talk to a health practitioner. Stress incontinence can often be successfully controlled through pelvic floor exercise, and overactive bladder symptoms may benefit through a combination of exercise and drugs. The main thing is to realize that you don?t have suffer alone; there a lot of leakage going on.

CHICAGO (AP) ? Patrick Fitzgerald has prosecuted mobsters, terrorists, a White House aide and two Illinois governors.
On Thursday, the former top prosecutor got a crack at Socrates. Yes, that Socrates, the Greek philosopher.
Fitzgerald, one of the nation's highest profile federal prosecutors until he recently entered private practice, represented Athens in a do-over of the 399 B.C. trial of Socrates on charges of corrupting the ancient city's youth and disrespecting its gods.
Socrates' legal counsel at the mock trial in Chicago ? part of a fundraising event for the National Hellenic Museum ? was no slouch himself.
It was Dan Webb, a high-priced lawyer who defended former Gov. George Ryan in a corruption case brought by Fitzgerald. Ryan eventually lost at a trial prosecuted by assistant attorneys working for Fitzgerald.
The result Thursday night? Jurors ? an audience of around 1,000 people ? found Socrates guilty by a narrow vote. They spared the philosopher death by hemlock, however, and called for a fine instead.
As U.S. Attorney in Chicago, Fitzgerald gained a reputation for getting defendants to plead out before trial. But he told The Associated Press by phone hours before Thursday's event that a last-minute plea deal with Socrates was out of reach.
"Socrates," he explained, "does not seem to be much of a compromiser."
In the 24 centuries since Socrates' trial and execution by poison hemlock, the prevailing sentiment has been that Athens railroaded the 70-year-old gadfly, who was fond of questioning bedrock Athenian assumptions about the world.
Fitzgerald, though, complained that the only extensive account of the trial is from Plato, a student and booster of Socrates.
"I don't think Athenians ever got a fair shake. Plato only gave one side of the story," he said.
Impiety was seen as an egregious crime in ancient Greece, Fitzgerald's co-counsel told jurors Thursday night, because it was thought that an individual's disrespect of the gods could invite their wrath in the form of plagues that would devastate the entire city.
"He dissed Zeus ... He called the Moon dirt," said Pat Collins, another former prosecutor representing Athens. "Messing with the gods brings real harm ... The gods have a memory, and they carry a grudge"
One judge presiding over the retrial was Richard Posner, who sits on the 7th U.S. Circuit Court of Appeals. Socrates was "a crank" who "encouraged the brats of Athens," he said, but wasn't a threat to society.
In that vein, another of the presiding judges said he could only see fit to impose a minimal fine in light of the jury's decision Thursday night.
"I'd fine him two bucks and let it go at that," said William Bauer, another federal judge.
The retrial wasn't meant to be a reenactment and so participating attorneys and judges weren't required to don togas or other period garb, Fitzgerald said gratefully earlier in the day.
"There are crimes against nature, too," he laughed. "That would be a crime against nature if we showed up in a togas."

OTTAWA ? Prime Minister Stephen Harper welcomed his Conservative caucus back to Ottawa Wednesday morning with a speech that outlined his government's plans to celebrate some important but potentially controversial milestones in the country's history.
With his five freshly appointed senators sitting in the front row in the Conservative caucus meeting room in Parliament's Centre Block, Harper vowed to keep a steady hand on the economy and to continue work on public safety issues. But he also said that Canada will soon "proudly remember" the centennial of the First World War beginning next year, the 200th anniversary of the birth of the Sir John A. Macdonald in 2015, and the 150th anniversary of Confederation in 2017.
"These milestones remind us of a proud national story rooted in the great deeds of our ancestors and in a centuries-old constitutional legacy of freedom," Harper said.
Though most historians would agree the First World War was an important event in Canada's development as a country, it was also one that saw the first major English-French split after Confederation.
Few officers in the Canadian army of the period spoke French and French Canada strongly resisted conscription in 1917, believing it to be imposed on them by an English Canada that was still, at the time, very much connected to an imperial Britain.
Modern-day Quebecers seemed to have little enthusiasm for last year's commemoration of the 200th anniversary of the War of 1812 - a war that was, technically speaking, between Great Britain and the United States, and a commemoration of the First World War may be used an excuse by some in Quebec to remind French Canada of the linguistic divide that exists today.
Notably, while Harper spoke in both French and English to his caucus when he spoke about his government's focus on the economy and on public safety, his remarks about commemorating the First World War and Macdonald's birthday were in English only.

Everstone Capital, among the biggest India-focused private equity houses, has held talks with Burger King, but the latter has not taken a final call on its India plans yet. The NYSE-listed burger chain is exploring a franchisee model in sync with its operations globally. "In the course of normal business, we continuously review our worldwide restaurant portfolio and make strategic decisions based on many factors including development opportunities, market conditions and restaurant profitability in various countries, including India. However, we do not have any further information to provide at this time regarding the Indian market," said Burger King spokesperson Miguel Piedra. Everstone Capital declined to comment on speculation.
Burger King had held discussions with DLF and Kishore Biyani's Future Group to enter the market some years ago. Everstone Capital, founded by two Goldman Sachs bankers Sameer Sain and Atul Kapur, controls Blue Foods, which claims to be India's largest restaurant company with brands like Copper Chimney, Noodle Bar, The Coffee Bean & Tea Leaf in its portfolio.
Burger King, now controlled by private equity group 3G Partners, is not alone in renewing interest in India's food and beverage (F&B) potential. Rival burger chain Wendy's, UK's noodle chain Wagamama, another London dim sum eatery Ping Pong, European pizza and pasta bar Vapiano and Jamba Juice are among those courting Indian partners. The entry of Seattle-based coffee chain Starbucks, after years of studying the market, has revived the global interest of other brands.Burger King, operating more than 22,000 outlets across the world, wants a fairly big entry into the market with the potential partner having to commit up to $100 million, said a second source who did not wish to be named. "There is a huge opportunity for brands like Wendy's and Burger King, considering nobody has dominance of the burger market here. It's still a lucrative market if you find the right partner and localize your business," said Arvind Singhal, chairman of Technopak Advisors, a retail consultancy firm.
MNC brands have taken a strong hold over India's robust eating out economy, riding on the broader middle-class consumption story. "Most successful Indian restaurateurs, barring Anjan Chatterjee of Specialty Restaurants, have banked on foreign tie-ups. This shows that Indian consumers now have the pocket and desire to experience 'new food'," said Siddharth Bafna, partner at Lodha & Co, an active advisor on F&B deals.
Jubilant FoodWorks (Domino's Pizza), Hardcastle Restaurants and Connaught Plaza Restaurants (McDonald's), Devyani International (Pizza Hut, Costa Coffee) and JSM Corp (Hard Rock, California Pizza Kitchen and Trader Vic's) are among the leading domestic restaurant groups that have partnered global brands to build market valuations. Yum Restaurants is possibly the only international chain to tap the Indian market directly, building a strong network of KFC and Pizza Hut outlets, again through sub-franchisees.

DETROIT (AP) - The left cleat of Detroit Lions defensive tackle Ndamukong Suh connected with Houston Texans quarterback Matt Schaub in his groin area in their Thanksgiving game.
It wasn't clear on replays Thursday whether it was intentional.
"I really don't have anything to say about that play or that person," Schaub said.
Suh was on his chest, taken down by an offensive lineman, when he extended his left foot below Schaub's belt. Suh didn't talk to reporters after the game.
Last year on Thanksgiving, Suh was ejected for stomping on the right arm of Green Bay offensive lineman Evan Dietrich-Smith and was suspended for two games.
Suh has been fined in previous seasons for roughing up quarterbacks: Cincinnati's Andy Dalton, Chicago's Jay Cutler and Cleveland's Jake Delhomme.

(Reuters) - A federal jury convicted a New York man on Tuesday of hacking into AT&T Inc servers and stealing the email addresses and other personal data of about 120,000 Apple Inc iPad users, a U.S. attorney in New Jersey said.
Andrew Auernheimer, 27, was convicted by a Newark, New Jersey, jury of one count of conspiracy to access the servers without permission, as well as one count of identity theft, said U.S. Attorney Paul Fishman.
The defendant faces a maximum five years in prison and $250,000 fine on each count. A co-defendant, Daniel Spitler, pleaded guilty to the same charges in June 2011 and is awaiting sentencing.
Prosecutors said Auernheimer and Spitler were affiliated with Goatse Security, a group of Internet "trolls" that tries to disrupt online content and services.
According to the government, the men used an "account slurper" that was designed to match email addresses with "integrated circuit card identifiers" for iPad users, and which conducted a "brute force" attack to extract data about those users, who accessed the Internet through AT&T's network.
The authors of the slurper then provided stolen information to the website Gawker, which published an article naming well-known people whose emails had been compromised, including ABC News anchor Diane Sawyer, New York Mayor Michael Bloomberg and current Chicago Mayor Rahm Emanuel, prosecutors said.
Tor Ekeland, a lawyer for Auernheimer, said his client was free on bail, and planned to appeal the verdict to the 3rd U.S. Circuit Court of Appeals in Philadelphia.
"We disagree with the prosecutors' interpretation of what constitutes unauthorized access to a computer under the Computer Fraud and Abuse Act," Ekeland said in a phone interview. He called the prosecutors' interpretation of that federal law "extremely expansive."
The trial lasted about one week, excluding a disruption related to Hurricane Sandy, and jurors deliberated for a couple of hours, Ekeland added.
AT&T has partnered with Apple in the United States to provide wireless service on the iPad. After the hacking, it shut off the feature that allowed email addresses to be obtained.
The case is U.S. v. Auernheimer, U.S. District Court, District of New Jersey, No. 11-00470.

INDIANAPOLIS (AP) ? Indiana Supreme Court justices weighing the legality of the nation's largest school voucher program made clear during a packed hearing Wednesday that the program's fate may rest on whether it primarily benefits students' parents or the religious institutions that run private schools.
The five justices prodded lawyers for both sides on the vouchers' consequences as they heard a constitutional challenge to the 2011 law under which more than 9,000 students have switched from public to private schools with help from state funds. The program was pushed by Gov. Mitch Daniels and is considered a model of conservative Republicans' approach to overhauling education.
Solicitor General Thomas Fisher, defending the law, said the vouchers allow parents to send their children to private schools they otherwise couldn't afford. He said parents, not the state, decide which schools receive public voucher money.
"Any money that ends up in the coffers of religious institutions is there at the discretion of the parent," Fisher said during the hour-long hearing.
But John West, the Washington attorney handling the court challenge pressed by the Indiana State Teachers Association, said that virtually all of the voucher money goes to schools whose primary purpose is to promote the teachings of their affiliated churches.
"Here, the state is directly paying for the teaching of religion," West said. "Many of these schools describe themselves as ministries of the churches they're affiliated with," he added.
The Indiana case is being closely watched because the vouchers are available to middle class students. Voucher programs in other states are generally limited to low-income students or those in failing schools. Conservative Republicans say the vouchers offer families more choices and will boost education by giving public schools greater incentive to improve. Critics contend the vouchers could cripple public schools by diverting desperately needed funds.
In considering the law, "The problem for me is 'the benefit of,'" said Indiana Chief Justice Brent Dickson, referring the wording of the state constitution, which precludes spending state funds for the benefit of religious institutions. Dickson and other justices repeatedly probed the nuances of that phrase's meaning ? including whether it applied to other government services or to state scholarships that help students attend church-affiliated universities like Notre Dame.
"Why is that permissible, and this is not?" Dickson asked. He also noted that in 1851, when the constitution was ratified, most of the state's schools were private and "the Bible was the textbook."
West argued that in those other cases, the state wasn't directly funding religious activity.
Justice Stephen David focused on whether the state will have shifted to a religious education system if the vouchers drain much of the resources from public schools.
"What if 97 percent of kids end up going to religious schools?" he asked Fisher. "That is altering the state of education."
"You still have decisions being made by the parents," Fisher insisted. He also said the question was hypothetical.
However, Fort Wayne Community Schools Board President Mark GiaQuinta said after the hearing that school vouchers already had damaged the state's largest public school district.
"We have lost $7 million of our budget to vouchers," he told reporters. The district's 2013 budget is $279 million.
The number of students using the vouchers has increased since the law went into effect. The system was championed by Superintendent of Public Instruction Tony Bennett. But Bennett's Democratic opponent, Glenda Ritz, defeated him in the Nov. 6 election after arguing that the vouchers and other changes were damaging education in Indiana.

ScienceDaily (Nov. 21, 2012) ? Studying the genetic variability of endangered species is becoming increasingly necessary for species conservation and monitoring. But, endangered species are difficult to observe and sample, and typically harbour very limited genetic diversity. Until now, the process of finding genetic markers was time consuming and quite expensive. These obstacles make the collection of genetic data from endangered animals a difficult task to fulfill. A research team led by Loun?s Chikhi, group leader at the Instituto Gulbenkian de Ci?ncia (IGC) and CNRS researcher (in Toulouse, France), has now contributed to change the odds when looking for diversity.
Taking advantage of cutting edge DNA sequencing methodology and the collaborations with the Sabah Wildlife Department in Malaysia, Rachel O'Neill's laboratory? (University of Connecticut) and a private company (Floragenex), they were able to identify the genetic markers for the Bornean elephant, an endangered species, using blood from very few animals. The results showed that Bornean elephants have very low genetic variability that can impact on their survival to a threatened habitat, but that variable genetic markers can still be identified.
The study now published in the journal PLOS ONE, besides contributing to the conservation of the Bornean elephant, opens new avenues for the conservation of other endangered species.
The Bornean elephant is a unique subspecies of the Asian elephant, with a quite distinct morphology and behavior. They are generally smaller than other elephants, with straight tusks and a long tail. Currently, there are around 2000 individuals, located only in the North of Borneo. It remains unknown how this population of elephants evolved to become so different and why its distribution is so restricted.
Despite being one of the highest priority populations for Asian elephant conservation, until now there were limited genetic tools available to study its genetic variability and none that had been specifically designed for this species. Now, in the work conducted by Reeta Sharma, a Post-Doctoral fellow in Loun?s Chikhi's group, for the first time DNA sequences that characterize the genome of the Bornean elephants, called genetic markers, were identified. The research team used two different DNA sequencing technologies that are fast and increasingly cheaper. This kind of technology has been used for common laboratory species such as mice and fruit-flies, but they are only now starting to be used on endangered and "non-model" species.
Until now, in order to determine whether the species still harboured sufficient genetic diversity it was necessary to look through huge regions of the genome, using classical genetics methodologies, or use markers developed for other species, with varying levels of success. This approach can become unsustainable for the endangered species, whose numbers have gone bellow a certain size for long time. The only study that previously had tried to analyse Bornean elephants, using genetic markers developed for other Asian elephants had found nearly no genetic diversity. The work now developed demonstrates that if the methodology can be applied to the Bornean elephant, it should be possible to find the needles we need, and not get stuck with the hay, i.e., to find variable genetic markers in many other species.
The DNA analysis done resulted from blood samples collected only from seven Bornean elephants from the Lok Kawi Wildlife Park (Sabah, Malaysia) and from Chendra, the star elephant of Oregon zoo (Portland, USA). But, the research team is confident that these DNA sequencing methods can be used to type genetically other biological samples, such as hair or faeces, easier to obtain from wild animals, even though blood or tissue samples are still necessary to identify the markers during the first steps.
Reeta Sharma, first author of this work, says: 'The methodology applied to identify the genetic markers for the Bornean elephant can be used in the future for studies on the genetic variability of other species or populations facing the risk of extinction.'
The Bornean elephants live in an environment where natural habitats disappear quickly, due to oil palm plantations and populations get isolated from each other. Having access to variable genetic markers will be crucial to identify populations that are isolated and genetically depauperate, and monitor them in the future.
The origin of these elephants in Borneo raises controversy that has been long discussed. The only study done on the basis of genetic data concluded that they had been present in Borneo for more than 300,000 years. This theory does not satisfy all researchers as there is lack of elephant fossils in Borneo to support it. Another theory is that the sultan of Java sent Javan elephants as a gift to the sultan of Sulu, who would have introduced them to Borneo.Lounes Chikhi suggests: 'The new genetic markers that we found may also allow us to unravel the mystery of the origin of these elephants in Borneo, and perhaps reconstruct part of their demographic history. This is very exciting '.
This research was carried out at the IGC in collaboration with the Laboratoire Evolution et Diversit? Biologique in Toulouse, and with the School of Biosciences, Cardiff University (UK), the Sabah Wildlife Department (Malaysia), the Danau Girang Field Centre (Malaysia), the Center for Applied Genetics and Technology, University of Connecticut (USA) and Floragenex, Inc. (USA). Research was funded mainly by Funda??o para a Ci?ncia e a Tecnologia (FCT), Portugal, and Laboratoire d'Excellence (LABEX), France.
